How Poor Posture Leads to Headaches

Headaches that build through the day and start at the base of the skull often begin in the neck — and respond to treating the neck rather than the head.

How Poor Posture Leads to Headaches

Not all headaches come from the head. A cervicogenic headache — one referred from structures in the neck — typically starts at the base of the skull, spreads up over one side, and gets steadily worse the longer you sit at a screen. People often treat it as a tension headache for years before anyone examines the neck.

Why the neck refers pain to the head

The nerves supplying the upper three segments of the neck share pathways with the nerve that carries sensation from the face and scalp. Your brain cannot always distinguish where the signal originated, so irritation in an upper neck joint or muscle is felt as a headache behind the eye or across the temple.

Sustained forward head posture is the usual aggravator. Holding the head a few centimetres forward of neutral multiplies the load the small muscles at the base of the skull have to manage, and they fatigue long before the working day ends.

A useful clue: if turning or holding your neck in one position reliably brings the headache on, and the pain is consistently on the same side, the neck is worth examining.

Signs it is coming from your neck

  • Pain starts at the base of the skull and spreads forward, rather than throbbing all over
  • Almost always the same side, without switching
  • Builds through the day and is worse after long periods at a screen
  • Accompanied by neck stiffness or reduced ability to turn your head
  • Little or no nausea and light sensitivity, which point more towards migraine

What actually helps

Painkillers manage the symptom but leave the driver in place. Treatment usually combines hands-on work to restore movement in the upper neck joints with specific strengthening for the deep neck flexors and the muscles between the shoulder blades, plus the workstation changes that stop you loading the area for eight hours a day.

Most people notice a meaningful change within a few weeks of that combination. Any sudden, severe headache unlike your usual pattern, or one with visual changes, slurred speech, or weakness, needs urgent medical assessment rather than physiotherapy.
