How we handle your information
Your health information is personal, and you are trusting us with it. This page explains what we collect, why we need it, who else ever sees it, and what you can ask us to do with it.

Who we are
CareForU Rehab is a rehabilitation clinic at 2762 Slough Street, Mississauga, ON L4T 1G3, offering physiotherapy, chiropractic care, massage therapy, acupuncture, naturopathy and custom orthotics. When we provide care we act as a health information custodian under Ontario's Personal Health Information Protection Act, 2004 (PHIPA). Information that is not health information — a general enquiry, for example — is handled under the federal Personal Information Protection and Electronic Documents Act (PIPEDA).
What we collect
We collect only what we need to answer you and to treat you well. Depending on how far you get with us, that can include:
- Your name, email address and phone number, and the service you are asking about, when you contact us through the booking form, by phone or by email.
- Anything you choose to write in the message field — which is why we ask you to keep confidential health details out of it.
- Your health history, the findings from your assessment, treatment notes and progress records, once you become a patient.
- Billing details needed to invoice you or to submit a claim on your behalf.
- Ordinary technical information your browser sends when it loads a page, such as your IP address and which pages you opened. Our web host records this to keep the site running and secure.
Why we use it
- To reply to your enquiry, and to arrange, confirm or reschedule appointments.
- To assess, plan, provide and document your care.
- To coordinate with other health-care providers treating you, where you have agreed to that.
- To bill you, or to submit a claim to your extended health insurer or another payer at your request.
- To meet the record-keeping and reporting obligations our practitioners owe to their regulatory colleges and to Ontario law.
Who else sees it
We do not sell your information, and we do not share it for marketing or advertising. We disclose personal health information only in these situations:
- To other providers in your circle of care, so they can treat you — unless you tell us not to.
- To an insurer, employer or other payer, when you have asked us to submit a claim or a report.
- To a service provider working on our behalf, such as our practice-management or IT provider, under an agreement that limits what they may do with it.
- Where a law, a court order or a regulatory college requires or permits us to disclose it.
How we protect it
Clinical records are held in a secured practice-management system, and access is limited to the people who need it to do their job. Paper records are kept locked. Everyone who works here is bound by confidentiality, and that obligation does not end when they leave.
One caution: email and web forms are convenient, but they are not secure channels. Please do not send confidential health details through the booking form or by email. Bring them to your appointment, or call us instead.
How long we keep it
Clinical records are kept for the minimum period our practitioners' colleges require — generally ten years after your last visit, or ten years past your eighteenth birthday if you were treated as a child. Enquiries that never turn into an appointment are kept only as long as we need them, then deleted.
Your rights
The record is ours to keep, but the information in it is about you. You may:
- Ask for access to a copy of your record.
- Ask us to correct anything inaccurate or incomplete.
- Withdraw or limit your consent to share it with others, though we will tell you plainly if that limits the care we can give.
- Ask us any question about how your information is handled.
Cookies and tracking
This website sets no advertising cookies and runs no third-party analytics or tracking scripts. Nothing you do on the site is used to build a profile of you or followed across other websites. If that ever changes, we will say so here first.
Changes to this policy
If we change how we handle your information, we will update this page and the date shown above it. The version published here is always the one in force.
